Even though I'm a highly disciplined goal-oriented person, I keep going back to old habits, gaining the weight I lose the hard way and skipping exercises for months on an end. I keep trying to stay on track but it doesn't always go right. I don't know what to do.

    Take it one day, one decision at a time. I have to put my exercise time on my calendar like an appointment and single the time out for that. I plan my meals one week in advance and make my grocery list from that, and pretty much only buy what is on my list. If you can find an accountability partner this will help immensely, Find some friend or family member to join you on your journey, and together you can help each other.
